Uttar Pradesh, Bihar, Madhya Pradesh, and Rajasthan together account for over 50 crore people — nearly 40% of India’s population. These states also have some of the highest PM-JAY beneficiary populations and the most critical need for the scheme. The Challenge

These states have fewer hospitals per capita compared to Southern and Western India. Many districts have limited specialist care — a patient needing cardiac surgery or neurosurgery in a rural district of Eastern UP or Southern Bihar may have no empanelled hospital within 100 km offering that specialty.

Major Referral Hubs

Uttar Pradesh: Lucknow (KGMU, SGPGI) and Varanasi (BHU) are the two primary hubs. Western UP patients also access Delhi hospitals via portability.

Bihar: Patna (AIIMS, PMCH, IGIMS) handles the vast majority of advanced cases. Bihar patients are among the highest users of PM-JAY portability — travelling to Delhi, Kolkata, and Vellore for super-specialty care.

Madhya Pradesh: Bhopal (AIIMS) and Indore serve western and central MP respectively. Jabalpur covers eastern MP.

Rajasthan: Jaipur (SMS Hospital, private chains) is the primary hub. Jodhpur (AIIMS) serves western Rajasthan.

Using Portability Strategically

PM-JAY’s portability feature is a lifeline for Hindi belt patients. If your district lacks a hospital for cancer treatment, joint replacement, or cardiac surgery, you can travel to any empanelled hospital in India. Major destinations include Delhi (AIIMS, Safdarjung), Chennai (major transplant centre), Mumbai (Tata Memorial for cancer), and Bengaluru (Narayana Health for cardiac).

Finding Hospitals in Your District

Even in underserved districts, there are usually empanelled hospitals for basic care — general surgery, maternity, eye care, and general medicine. Search your district on PMJAYHospitals.com to see what is available locally before deciding whether to travel.
