This means a family from Bihar can get heart surgery in Delhi, a worker from Rajasthan can get treated in Mumbai, and a student from Assam can access healthcare in Bengaluru.

How Portability Works

The PM-JAY IT system is centralised and accessible from any empanelled hospital across India. When you visit a hospital in another state, the Ayushman Mitra verifies your identity through Aadhaar-based biometric authentication — the same process as in your home state. Your eligibility and remaining coverage balance are checked in real-time from the central database.

The pre-authorisation request is processed through the same system. The hospital in the other state files the claim, and payment is settled between the insurance companies of the source state (where you are registered) and the destination state (where you are treated).

When Is Portability Useful?

Migrant workers: Millions of Indians work in states different from their home state. A construction worker from UP working in Gujarat can use his PM-JAY card at any Gujarat hospital.

Referral for advanced treatment: Your home district may not have hospitals offering the specialty you need. Joint replacement, cardiac surgery, cancer treatment, and organ transplant may require travel to a metro city. PM-JAY portability makes this possible without any extra paperwork.

Students: Students studying in another state can access healthcare using their family’s PM-JAY card.

Family visits: If you fall ill while visiting relatives in another state, your PM-JAY card works there too.

Documents Needed

The documents are the same as in your home state: Ayushman Bharat PM-JAY card (physical or e-card from ABHA app), Aadhaar card for biometric verification, and your medical reports if any. No additional documents or permissions are needed for inter-state treatment.

Tips for Using PM-JAY in Another State

Call the hospital before visiting to confirm they accept PM-JAY from other states. While all empanelled hospitals are supposed to accept it, practically it helps to confirm. If a hospital refuses your card citing “out-of-state”, call 14555 immediately — this is a violation of scheme guidelines.
