Getting cashless treatment at a PMJAY empanelled hospital requires specific documents for identity verification and scheme validation. Having the right documents ready before your hospital visit saves time and ensures a smooth admission process.

This guide lists every document you need — mandatory and optional — for availing cashless treatment under the Ayushman Bharat PM-JAY scheme.

Mandatory Documents

1. Ayushman Bharat PM-JAY Card (ABHA Card): This is your primary identification for the scheme. You can carry the physical card or show the e-card on your phone through the ABHA app or DigiLocker. If you do not have a card yet, the Ayushman Mitra at the hospital can help you generate one on the spot if you are eligible.

2. Aadhaar Card: Required for biometric authentication at the hospital. The Ayushman Mitra desk will verify your identity through Aadhaar-based fingerprint or iris scan. Carry the original — photocopies are not accepted for biometric verification.

3. Ration Card (if Ayushman card is not available): Your ration card can be used to locate your family in the PMJAY beneficiary database if you do not have the Ayushman Bharat card with you.

Recommended Additional Documents

4. Medical Reports and Prescriptions: Carry any existing medical reports, test results, X-rays, CT scans, MRI reports, or doctor prescriptions related to your condition. These help the hospital team understand your medical history and plan treatment faster.

5. Referral Letter (if applicable): If you are being referred from a primary health centre, district hospital, or another doctor, carry the referral letter. Some hospitals and certain surgical procedures require a referral for PMJAY claims processing.

6. Previous Discharge Summary: If you were previously treated at another hospital for the same condition, carry the discharge summary. This is especially important for follow-up treatments, re-admissions, or staged surgical procedures.

7. Any Government Photo ID: As a backup to Aadhaar, carry a voter ID card, PAN card, driving licence, or passport. Useful if there are issues with Aadhaar biometric authentication.

Documents You Do NOT Need

You do not need to carry income proof, BPL certificate, bank account details, or any payment. The treatment is completely cashless — no money is charged from the patient at any stage. The hospital settles the claim directly with the insurance company or state health agency.

What Happens at the Hospital

Step 1: Go to the Ayushman Mitra desk (usually near the hospital entrance or registration counter).

Step 2: Show your Ayushman Bharat card and Aadhaar card.

Step 3: The Ayushman Mitra will verify your identity through biometric authentication (fingerprint or iris scan).

Step 4: Once verified, the Mitra will check your remaining coverage balance and initiate the pre-authorisation request for your treatment.

Step 5: After pre-authorisation approval (usually within a few hours), you will be admitted for cashless treatment.

Tips for a Smooth Hospital Visit

  • Call the hospital before visiting to confirm they accept PMJAY for your specific treatment
  • Carry all documents in a folder — originals plus one photocopy set
  • Ensure your Aadhaar-linked mobile number is active for OTP verification
  • If biometric authentication fails (common in elderly patients or manual labourers with worn fingerprints), the hospital can use OTP-based verification instead
