Knowing your PM-JAY treatment history is important for several reasons — tracking your remaining ₹5 lakh balance, verifying that no fraudulent claims have been filed against your card, and maintaining a record of past treatments for future medical reference.

Method 1 — ABHA App

Download the Ayushman Bharat Health Account (ABHA) app. Log in with your registered mobile number. Navigate to “Treatment History” or “Claims History” section. You will see a list of all claims filed against your PM-JAY card — including hospital name, date, diagnosis, treatment performed, and amount claimed. This is the quickest way to check.

Method 2 — At the Hospital

Visit any PMJAY empanelled hospital and ask the Ayushman Mitra to check your treatment history on the PM-JAY portal. The hospital system shows real-time claim data including pending, approved, and rejected claims.

Method 3 — Call 14555

Call the PM-JAY helpline and request your treatment history. Provide your PM-JAY card number or Aadhaar number. The operator will read out your claim history and remaining balance.

What to Look For

Unfamiliar claims: If you see claims for treatments you never received, this is potential fraud. A hospital may have used your card details to file ghost claims. Report immediately to 14555.

Remaining balance: Calculate how much of your ₹5 lakh is left for the current year. This is critical if other family members need joint replacement, heart surgery, or other major procedures.

Claim rejections: Check if any past claims were rejected by the insurance company. If a legitimate claim was rejected, the hospital should have appealed. Contact 14555 for help with rejected claims.

Keep Your Own Records

Always keep copies of discharge summaries, operation notes, and billing statements from every PM-JAY hospitalisation. These serve as your personal medical record and evidence of treatments received — useful for future doctors, insurance claims, and dispute resolution.
